How to request for an ID replacement?
To replace a lost ID, kindly fill out this form and deposit payment amounting to P100 in any of the following banks:

After which, please email a scanned copy to rarelano@medicardphils.com or maricohermoso@medicardphils.com. Your new card will be delivered to your nominated address in 5-7 working days. Thank you.
